Aufgrund der Bindung von XBD173 am Translokatorprotein-18 in der mitochondrialen Membran wird die Aufnahme der Neurosteroid-Vorstufe Cholesterol verstärkt und erhöhte Mengen an Neurosteroiden gebildet. Diese Neurosteroide verändern die Funktion eines Rezeptors an der postsynaptischen Membran von Nervenzellen. Dies hemmt die Signalweiterleitung und führt auf Verhaltensebene zur angstlösenden Wirkung.
